I AGREE BY ENTERING INTO THIS EVENT I WILL PAY ALL FEES ASSOCIATED WITH THIS TOURNAMENT AND COMPLY WITH THE DECISIONS OF THE ADMINISTRATORS AND OFFICIALS. IF WEATHER CONDITIONS COMPROMISE ANY PLAYING TIME THE TOURNAMENT OFFICERS WILL NOT OFFER ANY REFUNDS OR RESCHEDULE GAMES.

ENTRY WAIVER FORM

This WAIVER agreement is made between TEAM: and SOUTHERN CALIFORNIA BASEBALL TOURNAMENT ASSOCIATION administration/officals, College School Districts and/or Boards, City Field Officials, and the MANAGERS/COACHES/PARENTS/GUARDIANS of the player(s) and participants whose names appear on the attached Team Roster and Registration Form.

Entry Waiver: In consideration for being permitted by the Tournament Officials to participate in activities and baseball games in which my Tournament Team is involved, I do hereby waive, release, and discharge any and all rights and claims for damages for personal injury, death or property damage which may occur as a result of participation in said activities. This release is intended to discharge in advance the Tournament Administration, Officials, and Officers of the SOUTHERN CALIFORNIA BASEBALL TOURNAMENT ASSOCIATION and the participating Teams, their officials/employees, and agents from any and all liability arising out of or connected in any way with my Tournament Team's participation in said activities even though that liability may arise out of negligence or carelessness on the part of the persons or entities mentioned above. It is understood that this activity involves an element of risk and danger of death or accidents including, but not limited to the possibility of being struck by a pitched, batted or thrown ball, knee or leg injuries from running or sliding or other such dangers that would be expected in the game of baseball and knowing those risks, I hereby, on behalf of my Tournament Team, assume those risks. I further agree to indemnify and to hold the above persons or entities free and harmless from any loss, liability, damage, cost, or expense, which my coaches and players may incur as the result of a death or any injury, or property damage that may be sustained while participating in said activities.

Emergency Medical Consent: I hereby consent that my team participates in the above activities, and I hereby execute the above Agreement, Waiver and Release on its behalf. I state that participating minor age players are physically able to participate in said activities and that I have signed parental consent for all minor aged participants. Furthermore, the tournament officials are given permission to apply first aid and/or emergency treatment as deemed necessary.

I HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN THE SOUTHERN CALIFORNIA BASEBALL TOURNAMENT ASSOCIATION, THE TOURNAMENT OFFICALS, ADMINISTRATION, ALL PARTICIPATING PARTIES AND MYSELF. I HAVE CHECKED THE MANAGER AGREES BOX OF MY OWN FREE WILL. I CAN PRINT A COPY OF THIS EXECUTED AGREEMENT FOR MY RECORDS BEFORE ELECTRONIC SUBMISSION IF I SO CHOOSE.
